Grant Completion Form

City of Holyoke Ordinance:
Sec. 2-509. - Grant reporting.
Any city department, of which has received grant funding by approval of the city council pursuant to Massachusetts General Laws, or any other authorizing criteria, shall within a reasonable period of time, but no later than 60 days from the conclusion of said grant, generate a detailed report which outlines the planned outcomes with the documented actual results. Said report shall be submitted to the city clerk for addition to the city council next agenda. Once city council has concluded its discussion of the report, it shall be posted on the city website for a minimum of two weeks for the general public.

  • Insert the grant fund number, name of grant, and the grantor
  • Insert the begin and end dates for grant, begin date is the date of final approval necessary for expenditures to begin (either grantor or City Council whichever comes last)
  • Give a brief explanation of the purpose of your grant and any constituencies involved
  • Enter total budget figure
  • Breakdown your grant fund actual expenses by budget line item. For personal services please provide employee names and total payments
  • Explain grant outcomes at end of grant period and their impact on our community and/or other constituencies
  • Use this space for any additional grant related comments
